Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses to examine the effect of treatment across trials with different levels of pragmatism.
Background
Pragmatic studies provide real-world evidence that can be used to make clinical decisions. The term "pragmatic", however, is used inconsistently and its definition and measurement require further clarification. Pragmatic trials should be designed to inform policy and clinical practice decisions, rather than to prove a physiological or clinical hypothesis. A pragmatic trial should try to be as close as it is to actual clinical practices which include the recruitment of participants, setting, design, implementation and delivery of interventions, determining and analysis outcomes, and primary analysis. This is a major distinction from explanation trials (as described by Schwartz and Lellouch1) which are designed to provide more complete confirmation of an idea.
Trials that are truly pragmatic should avoid attempting to blind participants or the clinicians in order to result in bias in the estimation of the effect of treatment. The trials that are pragmatic should also try to attract patients from a variety of health care settings to ensure that the results are generalizable to the real world.

Furthermore the focus of pragmatic trials should be on outcomes that are vital for patients, such as quality of life or functional recovery. This is especially important in trials that require the use of invasive procedures or could have serious adverse consequences. Despite these requirements, many RCTs with features that challenge the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all types. This can lead to false claims of pragmatism, and the use of the term must be standardized. The development of the PRECIS-2 tool, which offers a standard objective assessment of practical features is a great first step.
Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by showing how an intervention could be integrated into routine treatment in real-world contexts. This is distinct from explanation trials, which test hypotheses about the cause-effect connection in idealized conditions. Consequently, pragmatic trials may be less reliable than explanatory trials and may be more susceptible to bias in their design, conduct and analysis.
